Required forms and immunization documents
All students at the traditional campus are required to complete TB screening questionnaire and Consent to Treat form online in the Student Health Portal. Students must submit proof of two vaccinations for measles, mumps and rubella (MMR). Failure to submit these documents to Health Services will prevent students from registering for subsequent semesters. For more information, contact Health Services at (573) 875-7423.
